Performs tasks and responsibilities within the framework of health counseling and provision of supportive and restorative care under the direction of the RN or MD; assigned to attend to the medical needs of individuals in the Day Habilitation program; includes, but is not limited to direct administration of gastrostomy tube feedings (upon successful completion of OPWDD course), suctioning airways (under supervision of RN) medication administration, first aid and emergency response; provides supervision and medical oversight while accompanying individuals during transit to/from their homes and Day Habilitation program.
Provides supervision and medical oversight for individuals while on community opportunities; transports individuals to job opportunity sites or other community-based activities; assists individuals in achieving success and satisfaction in their community-based endeavors; completes all requested medical and program documentation (i.e. Daily Records; Program notes, etc. ) related to the care of the individuals

This position requires regular and substantial unsupervised and unrestricted physical contact with people receiving services and, therefore, is subject to successful completion of fingerprinting/criminal background check.
Ensures that work-related safety procedures are in place and are being adhered to by staff. Observance of an unsafe act or a potentially hazardous work environment must be immediately reported to supervisor and/or safety committee.
Personal outcome measures training is required prior to completion of evaluations and assessments.
